- Medical and Intake Information: Health history, medications, psychological background, and other details required for participation in psilocybin services.
- Session Notes: Observations, reflections, and anonymized time-stamped notes recorded during or after preparation, journey, and integration sessions.
- Administrative Information: Payment details, scheduling information, and documentation required for service compliance.
This information is collected directly from you and stored in a secure, encrypted database.

How We Use Your Information
Your information is used solely to support your psilocybin service process and to ensure compliance with Oregon law. Specifically, ClearHaven may use your information to:
- Communicate with you about your sessions, scheduling, and follow-up care.
- Assess eligibility for psilocybin services in accordance with OHA requirements.
- Coordinate logistics with your chosen licensed psilocybin service center.
- Maintain accurate client records as required by Oregon Psilocybin Services regulations.
- Provide personalized preparation, facilitation, and integration support.
Your information will never be sold, rented, or shared for marketing or commercial purposes.

Information Sharing and Disclosure
ClearHaven maintains strict confidentiality standards. Information you share is only disclosed in the following cases:
1. To the Chosen Psilocybin Service Center:
Limited personal and medical information necessary for compliance with OHA regulations will be shared with the licensed service center where your journey takes place.
2. To the Oregon Health Authority (OHA):
Certain anonymized or de-identified information may be submitted as required for public health reporting or data collection. This will never include personally identifiable information without your explicit consent.
3. With Your Express Permission:
If you choose to provide written authorization, ClearHaven may share information with another provider, therapist, or support person you designate.
4. When Legally Required:
ClearHaven may be required to share limited information if mandated by law, such as in cases of imminent harm to self or others, suspected abuse, or court orders.

Confidentiality of Session Notes
All notes, reflections, and records created during your preparation, journey, and integration sessions are stored securely and treated as confidential clinical documentation.
- These notes are never shared outside of ClearHaven without your written consent.
- You may request a copy of your notes at any time.
- Facilitators are required by law to retain these records for the duration specified by OHA but will not use them for any purpose beyond client care or compliance.

Data Storage and Security
ClearHaven uses encrypted, password-protected digital storage for all electronic records and locked, secure storage for any physical documents. Access is restricted only to authorized personnel directly involved in your care.
We take reasonable administrative, physical, and technical safeguards to protect your information from loss, theft, unauthorized access, disclosure, alteration, or destruction.

Client Communication
Your contact information (email, phone number) will be used only for communication related to:
- Scheduling and logistics for psilocybin services.
- Sharing information relevant to your preparation, journey, and integration process.
You may request at any time to limit or modify how you receive communications from ClearHaven.

Consent to Collect and Store Information
By engaging with ClearHaven’s services, you acknowledge and consent to the collection, storage, and use of your personal information as described in this Privacy Policy. You may withdraw consent to store or use your data (except where retention is required by law) by contacting ClearHaven in writing.
